Breast implant revision can arise from several factors, each requiring meticulous surgical attention and thorough analysis. The main causes include:
- Implant rupture: Although breast implants have an estimated lifespan of 20 years, rupture is not the only reason for revision. In many cases, ruptures are detected incidentally during a breast ultrasound or MRI scan, without the patient having experienced any obvious symptoms. However, if pain, a change in shape or discomfort occurs, revision becomes necessary.
- Poor results from the original surgery: Some patients come to our clinic after an unsatisfactory previous breast surgery. Unaesthetic results can be the consequence of an incorrect choice of implant, a poor surgical technique or an inappropriate initial recommendation. A thorough analysis of the situation is crucial in these cases, as every detail — from the condition of the old implants to a physical assessment of the breast — must be carefully reviewed.
- Changing the implant volume: Whether to increase or decrease the size of the implants, this is one of the most common reasons for revision. This procedure may involve modifying the implant pocket to fit the newly desired size.
- Capsular contracture: Capsular contracture is a complication in which the tissue capsule surrounding the implant hardens and tightens, causing distortion of the breast shape, pain and discomfort. Although it can have several causes, one of the most common is bacterial contamination of the implant surface. In these cases, a capsulectomy (complete removal of the capsule) is necessary, along with stabilising the pocket before inserting new implants.
- Changes in breast shape: Over time, a woman’s breasts undergo natural changes, even with implants in place. Factors such as ageing, weight fluctuations, pregnancy and breastfeeding can cause the breasts to sag. In these cases, implant revision is often combined with a mastopexy (breast lift) to restore the shape and firmness of the breast.
